Dvora Studio and Headup have confirmed that The Coma 3: Bloodlines, the final mainline installment in the Korean survival horror series, will launch on April 30, 2026, across PlayStation 4, PlayStation 5, Xbox One, Xbox Series X|S, and PC through Steam, Epic Games Store, and GOG.
The Coma 3: Bloodlines ends the series that began with The Coma: Cutting Class in 2015 and continued through the 2017 remaster The Coma: Recut, the 2020 sequel The Coma 2: Vicious Sisters, and the 2024 spin-off The Coma 2B: Catacomb. Where Vicious Sisters pulled players out of the emptied school grounds and had them explore the neighborhoods around Sehwa, Bloodlines has players returning to campus to witness the finale of everything the previous entries had been pointing toward.
The setting is set in Seoul, specifically a district whose public-facing institution, Sehwa Private High School, doubles as cover for the Sehwa Foundation, a group devoted to researching the cursed underworld of history and fixated on a mirror dimension they call the Coma. Alongside them are the Ghost Vigilantes, another group that treats the same place as an object of exploration and works to suppress the dangerous elements within it. The final secret of Sehwa High School drives the game’s central narrative, while the history of the Ghost Vigilantes unfolds alongside it. Wraiths from the past have appeared in the present day, and the Coma has once again become a nightmarish realm.
Gameplay rotates between three playable characters, each with their own strengths and weak points, and all three will be recognizable to returning fans of the series. The game’s signature run-and-hide loop still centers around stealth and concealment, forcing players to evade a mix of shadow monsters, wraiths, and the franchise’s iconic puppets. Four new bosses are confirmed, each differing in how it pursues and strikes at the player, and the game’s level design uses smaller, more confining areas. Within the Coma, dying means a living intruder’s body is cut off from their spirit with no return.
